Social Prescribing Link Worker
Join our fantastic Social Prescribing Team, based in The Leys, supporting people to access community support and activities. In partnership with the South East Oxford Health Alliance (SEOxHA), our team of 3 Social Prescribers support referrals from health centres in the Leys, Cowley and Donnington.
Duration: Permanent
Hours: Full time (37.5 hours per week)
Salary: £30,523.50 p/a
